Cohome was founded to provide adults with disabilities meaningful opportunities to fully engage with their communities and lead fulfilling lives. Our person-centered programs nurture self-advocacy, independence, and meaningful connections.

Ours is a story of family

Nathaniel Diskint, inspired by his younger brother Jeremy who has Down Syndrome, sought to create a vibrant and inclusive housing program. In 2017, Nathaniel and the Diskint family founded Cohome, a 501(c)3 not-for-profit organization to advocate for inclusion through an innovative coliving model. Today, the Cohome brand has grown to include a private company creating independent living opportunities for adults with disabilities through value-add public and private partnerships as well as Journey Services, a disability supportive services provider agency. Our family has also grown to include many unique and wonderful residents, families, volunteers, employees, and supporters whose efforts make Cohome the joyous community it is today.

 FAQs

  • Yes, we do have an waitlist at this time. Some people choose to join our social club in order to get a jump start to being involved in the community. We also recommend that you join our mailing list and follow us on social media to stay connected and learn about special events.

  • ■ All housemates must be 18 years or older.

    ■ All our housemates are required to be safe to themselves and others at all times.

    ■ Individuals with insufficient income may require a guarantor when signing the lease and program documents. That said, many of our individuals find it especially helpful to be registered with the Division of Developmental Disabilities in order to receive both a housing voucher and a support budget.

    ■ Cohome operates its housing program in compliance with federal and state housing laws.

  • ■ Life Skills Mentors are provided by Journey Services, a DDD-approved provider agency dedicated to helping individuals with disabilities live their best lives. Please connect with Journey Services directly to learn more at journey@cohome.org.

    ■ Absolutely. If you live on your own or with family, our Life Skills Mentors can help you develop independent living skills and support your unique journey. Please connect with Journey Services directly at journey@cohome.org.

  • Yes! The Social Club is open to members who do not live with us but want to join this amazing, inclusive group to hang out, make friends, go on adventures, and develop independent living and social skills. Please connect with a team member to learn more and sign up at admin@cohome.org.

  • Cohome is a mission-based Benefit Corporation that also supports Cohome, Inc, a tax exempt organization with 501(c)3 status. All donations made go directly to supporting Cohome, Inc’s important work creating and advocating for inclusive opportunities for adults with disabilities. Cohome, Inc’s tax ID number is 82-1500888.

  • We are so proud of our program and the wonderful inclusive culture we've created. It takes a lot of time, attention, and resources to make inclusion happen. Therefore, donations for nonspecific general operating funding can make the greatest impact on our programs.

  • Absolutely! A visit is part of the application process. Please complete an application to get started. We can also make special appointments for prospective funders, sponsors, and corporate partners.
